New York State Department of Health Releases Latest List of Enforcements

Recently the New York State Department of Health released its latest update of fines against New York nursing homes. Most of the more recent violations resulting in fines have been covered here in previous blog posts. Unfortunately, many of the original violation dates leading to the fines listed on the enforcement log pre-date the oldest records currently listed on the DOH website. This makes it difficult to determine the violations that prompted the Department to fine these various websites. However, generally, the Department of Health will fine a nursing home if violations at a facility rise to Levels 3 or 4 on its ratings scale (actual harm for Level 3, Immediate Jeopardy for Level 4). The nursing homes in the New York metro area receiving fines for the past year (Q1 2012 through Q1 2013) are below. Instances for which we have done a previous blog post are linked.

  • Northern Riverview Health Care Center, Rockland, $16,000
  • Ocean Promenade Nursing Center, Queens, $4,000
  • New York State Veterans Home at Montrose, Westchester, $8,000
  • Avalon Gardens Rehabilitation & Health Care Center, Suffolk, $4,000
  • Hopkins Center for Rehabilitation and Healthcare, Brooklyn, $4,000
  • Somers Manor Nursing Home, Westchester, $14,000
